Purpose of Role: The post holder will support service users and their carers through a range of aspects during their treatment stay.
Key Responsibilities:
1. Manage a workload as agreed by the Lead Social Worker and MDT.
2. Undertake work as agreed in relation to The Mental Health & Mental Capacity Act.
3. Prepare and present reports for Mental Health Review Tribunals and Managers Hearings.
4. Prepare and present Social History reports and reports for Clinical Reviews.
5. Attend relevant multi-disciplinary meetings and liaise with community social workers, other agencies, and professionals.
6. Engage in 1-1 and group work with service users and maintain up-to-date records in clinical notes of all service user contacts.
7. Liaise with service users' families/carers and other statutory/voluntary agencies.
8. Undertake child visit and home visit risk assessments, making recommendations to the MDT.
9. Lead on planning for service users' discharge and aftercare arrangements, including aspects of safeguarding and multi-agency public protection arrangements.
10. Identify, visit, and assess resources that may be considered appropriate for service users moving into the community and/or other residential settings.
11. Adhere to MAPPA processes, making referrals and attending MAPPA meetings where appropriate.
12. Undertake S42 enquiries as directed by the Local Authority.
13. Lead on the day-to-day coordination of safeguarding.
14. Attend and take an active part in Social Work team meetings.
15. Regularly attend prearranged supervisions with Lead/Senior Social Worker with prepared agenda items.
16. Demonstrate an awareness of mental health and childcare legislation in relation to all aspects of work within St Andrew's Healthcare.
17. Undertake mandatory training and attend any training as advised by the social work supervisor to maintain professional registration with SWE.
18. Deliver Social Work based training within the Division/Hospital as required.
19. Complete the Social Work post-qualifying ASYE.
