Job Summary As a Prescribing Pharmacist, you will play a key role in medication management, ensuring safe, effective, and evidence-based prescribing for our patients. Your primary responsibilities will include prescription signing, medication reviews, reconciliation, and conducting safety searches to optimize prescribing practices and enhance patient care. You will work closely with GPs, nurses, and other healthcare professionals to support medication safety, chronic disease management, and prescribing initiatives, ensuring compliance with clinical guidelines and improving patient outcomes.
