Job responsibilities The AAA Screening specialist nurse will run nurse led screening clinics in GP surgeries and other community based venues throughout Leicestershire and Rutlandcarrying out ultrasound scans, and providing health assessment and lifestyle advice/counselling to men who screen positive for abdominal aortic aneurysm, below the referral threshold.Duties will include: Organise follow up for patients with positive scans including referral into Vascular surgery where appropriate. Assess and counsel men at specific points in the screening process, to give advice on changes in lifestyle and where appropriate make referrals to local health and welfare services. This will be in accordance with the AAA Screening care pathway. To provide support, advice, and referrals (if appropriate) to ensure individuals who screen positive for AAA understand the issues and consequences associated with AAA management and surveillance. This includes the risks of being in the surveillance programme and current treatment options. Provide advice, education, support and information to patients as required and involve patients in planning and delivery of care. Establish and maintain effective communication with patients. Facilitate effective communication and liaison with other disciplines as necessary to meet the needs of the patient especially in relation to on-going care. To be a point of contact for any man who wishes to discuss any concerns related to their clinical findings. Participate in the delivery of health optimisation initiatives within the local programme.
