The Assertive Outreach Team (AOT) in Yeovil is seeking a dedicated Community Support Worker to become an integral part of our close-knit, multidisciplinary service. Who We Are : AOT provides specialised mental health support to individuals with severe and enduring mental health difficulties in the South Somerset area. We focus on engaging with those who have found it challenging to access standard community mental health care, offering tailored support to reduce the risk of relapse and inpatient admission. Main duties of the job As a Support, Time and Recovery Worker within the Assertive Outreach Service, you will: Collaborate closely with a multidisciplinary team, including professionally qualified colleagues and other Support Time and Recovery (STR) workers, to ensure accurate updates on the treatment and recovery progress of individuals under our care. This includes both inpatients and those receiving home treatment. Actively participate in training sessions, team meetings, care plan reviews, and supervision to support ongoing professional development and service excellence. Provide consistent and regular support to help individuals engage with their agreed care plans and achieve their recovery goals. This involves delivering evidence-based interventions under supervision, such as graded exposure for anxiety, behavioural activation for depression, and strategies for emotional regulation. Promote independent living by supporting individuals in the community with practical assistance and empowering them to maintain dignity and independence. Build and maintain therapeutic relationships with patients within appropriate and professional boundaries, fostering trust and a focus on recovery. Work collaboratively with keyworkers to provide tailored support for daily living, aligned with the goals outlined in each individual's care plan.
