Windsor House Group Practice is looking to recruit a Practice Nurse to work between 3 to 4 days per week. We are looking for someone who enjoys working in a patient-facing role and has good communication and organisational skills.
The post-holder will work in the practice clinical team delivering high-quality healthcare to registered patients within our large training practice (approx. 19,300 patients). The post-holder will manage a case load, lead on agreed clinical targets and service developments, and provide support to Partners, GPs, nurses, registrars, and other clinicians within the practice.
Main duties of the job
The practice nurse will undertake a range of nursing assessments and provide appropriate care/treatment in conjunction with GPs and HCAs or independently as appropriate for the need, according to practice policy and protocols. The practice nurse will strive to improve standards of care and be involved in identifying practice population needs. The successful candidate will work at Windsor House Group Practice to enhance nursing services within the practice. Chronic Disease management is a key part of the role, and knowledge of the GMS contract, e.g., The Quality and Outcomes Framework, is a vital element of the post. Additionally, the successful candidate will be encouraged and supported to undertake any further training required to ensure appropriate role development.

Windsor House Group Practice is a forward-thinking and high-achieving training practice located in the Leeds area (Morley, Churwell, and Drighlington) within easy access of the M62. The Practice is well-established and respected locally. We pride ourselves on providing high-quality care and innovation in the development of local services through our Primary Care Network (PCN).
The practice operates from three well-equipped premises. We provide a number of enhanced clinics, including joint injections, contraceptive implants, coils, ECGs, 24-hour BPs, and spirometry.
Our practice team consists of seven partners, five salaried GPs, four Practice Nurses, three HCAs, and an efficient reception and administration team. We are a GP Training Practice and currently have ten GP registrars.

Job responsibilities
1. Assist GPs to help deliver effective implementation of management of patients with chronic diseases as detailed by the Quality and Outcomes framework.
2. Contribute to the development of policies/protocols/guidelines within the practice.
3. Aid the practice team with the implementation of new initiatives in the delivery of chronic disease management services.
4. Formulate reports as required for practice management.
5. Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.
6. Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working.
7. Promote the continuing professional development of yourself and other nurses & HCAs within the practice.
8. Recognise the opportunities for working in partnership with the Primary Health care team.
Clinical Responsibilities
1. Work alongside Health Care Assistants and Phlebotomists to help develop their skills.
2. Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Frameworks.
3. Undertake wound care/management/dressing techniques.
4. Carry out ear examination and syringing, following appropriate training and assessment.
5. Undertake family planning assessments, give advice, and refer patients appropriately - following appropriate training.
6. Perform cervical screening and promote breast awareness - following appropriate training.
7. Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation, and disposal of vaccines and drugs.
8. To undertake such duties in specific chronic disease management, e.g., diabetes, respiratory, epilepsy, CHD, etc.
9. Contribute to the maintenance of the practice diseases registers.
10. Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
11. Provide an adult and childhood immunisation service working to Patient Group Directions - following appropriate training if required.
12. Deliver travel health care, including vaccinations and advice for patients - following appropriate training if required.
13. Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local referral procedures.
14. Ensure that current evidence-based approaches to patient care are standard practice.
15. Apply infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages, and report and treat sharps injuries.
16. Undertake first aid and management of emergencies, e.g., burns, haemorrhage, hypo/hyperglycaemic emergencies/asthma attacks.
17. Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques.
Person Specification
Experience
* Experience of working within a general practice.
* Understanding or experience of audits.
* Experience of managing clinics in a Primary Care setting.
* Experience in the development of General Practice services.
Qualifications
* Registered nurse or registered general nurse.
* Long-term condition qualification (e.g., respiratory medicine or diabetes diploma, etc.).
