Job summary A Care Navigator responds to requests for GP appointments, made by patients either on the phone or in person. They will triage the patient non-clinically, and signpost to the appropriate service or person, which may not necessarily be the GP. They will work in a reception hub or on front desk reception. We are looking to cover Mondays 12.30-6.30 Tuesdays 12.30-6.30 Wednesday day off. Thursday 12.30-6.30 and Friday 12.30-6.30. (24 hours) Main duties of the job This is a challenging but very satisfying role. Patients rely on our care navigators to listen, advise and point them in the right direction. Often, patients themselves do not know what the need in terms of their health and it is up to a trained navigator to support them. About us East Trees Health Centre is based in inner city Bristol. We are a training practice with a list size of 15700 patients. Our practice team comprises of 4 partners, 7 salaried GPs, a paramedic, nurse practitioner, physio, podiatrist,dietitians, care co-ordinator, care navigators, administrators, nurses and pharmacy team. We are a very friendly, close knit team and have regular staff and team meetings to ensure communication is consistent.
