Role Summary
Castle Place Practice are looking for a Care Coordinator to join our busy admin and reception teams, providing support to our Patients, GP's, Practice manager and other members of the Primary Health Care team. This is an essential role ensuring patients receive the highest quality of care and helping to coordinate various work streams, provide a welcoming and friendly professional service to general members of the public and all staff and visitors to the practice.

Previous experience of working within the GP Practice or other health care provider would be desirable. You must be organised, self-motivated, forward thinking with excellent customer service skills. It is important that the candidate is empathetic towards patients using the Practice and can remain calm under pressure and deal with patient queries whilst answering calls and dealing with GP requests, be assertive and able to multi task and deal with a high volume of patients in reception whilst responding to other demands. Knowledge of medical terminology would also be an advantage.
This role is a wide and varied one and the successful candidate will work closely with the patient and a multi-disciplinary team of clinicians to co-ordinate their healthcare needs, directing them to the appropriate service ensuring they get the most suitable care from whatever health or social care provider is appropriate.
You will also be the first point of contact for the care homes covered by Tiverton PCN and you will build and maintain good working relationships with care home staff. You will also be allocated a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.

Care Homes
1. Main point of contact at the Practice for Care Homes
2. Check patients in care homes have up to date medication reviews and care plans and keep track of the numbers, Liaise with GP, Paramedic & Pharmacy Team
3. Create and maintain a rolling programme of GP face to face reviews, at least annually for residents. New residents should be seen by a GP as a matter of priority on these visits.
4. Make GP/Paramedics Aware of LTC’s that are required
5. Receive weekly ward round list’s from the care homes and cascade to the relevant clinicians.
6. Monitor Task inboxes
7. Monitor waiting lists for Coils/implants & Minor Ops
8. Vaccination waiting lists & booking of appointments
9. Run & book Diary recalls
10. Three-month cancer telephone reviews
11. Monitor RDUH discharges daily add medication changes to Pharmacy screens & book required follow up appointments
12. Monitor Care Coordinator mail inbox
13. Supervision of Castle Place Practice volunteers
14. Retinal screening document upload/Coding (Desp) Check ADHD Florey inbox
15. Monitor Care Coordinator Accurx inbox
16. LTC booking line open daily am – pm daily
17. Registrations/Death process for Care Home Patients
18. Support other administration teams as required
QOF & Recall – Long Term Conditions (LTC)
19. Assisting Data Lead on QOF and Recall (LTC’S)
20. Data integration · Liaising with the Nursing Lead regarding waiting lists
21. Conducting patient searches in EMIS
22. Managing monthly diary entries & booking patient appointments, add future diary dates as required
Learning Disabilities
23. Maintaining the Learning disability patient list with support from Clinicians/Data lead
24. Inviting patients for their physical health checks & arranging follow ups as required

Mental Health
