We are looking for an enthusiastic, professional and organised Patient Co-ordinator to join our friendly team as soon as possible. The role is for 30 hours per week. The successful candidates must be able to work flexibly between the hours of 8.00am and 6.30pm Monday to Friday.
Main Duties of the Job
Reception & Administrative Duties
Ensure an efficient front desk service is provided to patients and any other visitors attending the practice.
Processing telephone requests for appointments, visits and general enquiries; ensuring calls are signposted correctly.
Job Responsibilities
The following are the core responsibilities of the Patient Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Administration
1. To have a thorough knowledge of all Practice procedures.
2. To work in accordance with written protocols.
3. Filing patient data into medical records.
4. Photocopy as requested.
5. Receive and make phone calls as required, ensuring accuracy of detail and prompt delivery of messages.
6. To process POLAR, registration, emails and tasks on a regular basis.
7. To deal with patients complaints within their competency.
8. To liaise with other members of the primary health care team.
9. To attend staff meetings and QUEST training and complete on-line training.
10. Contact patient via telephone call or SMS to cancel appointments.
11. Ensure all relevant scanning is completed daily for all prescription queries.
12. To assist with the administrative/Data functions of the practice when cover is needed.
Reception
1. Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice.
2. Handing completed repeat prescriptions and other patient information to the patient and checking D-O-B, names and addresses.
3. Explain practice arrangements to new patients and temporary residents, completing necessary paperwork.
4. Be able to cover all reception positions.
Appointments
1. Process requests for on the day and future appointments from patients by telephone and in person.
2. Deal with all general enquiries, explain procedures to patients and make new and follow-up appointments.
3. Ensure that patients needing urgent appointments are seen or added to triage.
4. Enter requests for home visits onto the visit screen, ensuring careful recording of all relevant details and where necessary refer to the Duty Doctor.
Computer
1. Update patient contact number on the patient record.
2. Action repeat prescription requests and ensure that they are ready for collection within 48 hours or send EPS.
Stock
In addition to the primary responsibilities, the Patient Coordinator may be requested to:
1. Assist with queries regarding safeguarding matters and refer appropriately.
2. Provide support/cover to other teams within the practice as part of Swadlincote Surgery's Business Continuity.
3. Flexibility to cover Monday-Friday during periods of holiday and staff absence.
