We have 2 part-time band 4 community nurse associate posts in the Blandford, Gillingham and Blandford locality District Nursing teams. You will deliver planned care to patients in the community setting.
Will support individual development and training needs for the role and responsibilities. To work effectively within a large team of mixed skills when caring for complex patients in the community setting. Effective communication is key to this role and being aware of multi-disciplinary working.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job
1. To provide a high-quality person-centred approach to care delivery within your sphere of practice always considering patients' privacy, dignity and safety.
2. To deliver planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council (NMC) and in accordance with Trust policies and protocols.
3. To demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.
4. Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence-based/client-centred principles to undertake a range of delegated tasks independently.
5. Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalate to senior registered practitioner as appropriate.
6. To promote health and prevent ill health in service users, contributing to integrated care.
7. To take responsibility for planned/defined tasks as required.
8. To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Registered Nursing Associate role.
9. Undertakes delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.

Person Specification
Knowledge skills and training
* Registered Nursing Associate on the NMC register.
* Understand and act in line with NMC professional standards.
* Ability to develop effective and appropriate relationships with patients, their families, carers and colleagues.
* Ability to communicate effectively (written, verbal, and nonverbal communication) with patients, relatives/carers and the multidisciplinary team.
* Understanding of the scope of the role of the Nursing Associate in the context of the team and organisation, and how the role may contribute to service development.
Job specific experience
* Ability to organise and prioritise own delegated workload.
* Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
* Experience of providing and receiving complex, sensitive information.
Communication
* Ability to develop effective and appropriate relationships with patients, their families, carers and colleagues.
* To be aware of own emotional and mental health needs when caring for patients during distressing and emotional circumstances such as terminally unwell patients, difficult family circumstances and safeguarding issues.
Knowledge
* Contribute to the assessment, planning, implementation and evaluation of patient/service user care.
* Undertake additional champion/link role relevant to speciality.
