This is a part-time Social Worker role at Elysium Healthcare's St Mary's Hospital in Warrington. The successful candidate will be responsible for managing a complex caseload, completing social circumstances reports, risk assessments, and contributing to ICR and CPA meetings. They will support service users with Acquired Brain Injuries, progressive neurological conditions, and acute Mental Health problems to make plans for their future.
Main duties of the job
As a Social Worker, you will be responsible for referrals to Local Authorities, educating hospital staff on safeguarding matters, completing annual social care reassessments, and facilitating access to welfare rights, housing, advocacy, and other support services. You will work as part of a multidisciplinary team to ensure service users are supported to make decisions and access the community, with the goal of promoting recovery and independence.

With previous experience from the NHS or private care, you can join the multidisciplinary team at St Mary's in Warrington which has a strong emphasis on teamwork, and where you will provide care for service users with Acquired Brain Injuries (ABI), progressive neurological conditions, and acute Mental Health problems.
As a Social Worker you will manage a complex caseload and be responsible for social circumstances reports, risk assessments, and will actively contribute to ICR meetings and CPA meetings, supporting service users to make viable plans for their future.
You report to the Lead Social Worker, and will manage internal investigations and liaise with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can see your career grow and you achieve your career aspirations.

As a Social Worker you will be:
* Referrals to Local Authority in line with safeguarding and assessments
* Educating Hospital staff about relevant safeguarding matters.
* Completing a full annual social care reassessment within the CPA format.
* Facilitating access to welfare rights, housing, advocacy services and associated support services
* Participating in 1:1 supervision
To be successful in this role, you'll need:
* To have a Social Work England registration
* A Social Work degree
* Experience within a similar environment
