To deliver interventions as delegated by therapists, to support patients toachieve their optimum level of independence and quality of life in the most appropriate setting for the individual.
To assist patients to overcome their difficulties, build confidence, and improve or maintain functional ability and independence in everyday tasks under the supervision of a therapist.
To work on a rota system across 7 days.
To participate in multi-disciplinary teams promoting the role of rehabilitation and taking an active part in team projects.
To organise and manage a delegated caseload; obtain service data, analysewhere appropriate and bring forward ideas for service development.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and motivated individual to work as part of the Community Therapy Team to support a discharge to recover and assess pathway in North Powys.
The successful candidate will work under the direction of the Occupational Therapists and Physiotherapists, providing direct and indirect patient care in order to support patients and their families within their own home environment.
The team is committed to facilitating rapid hospital discharge and maximising independence within a patient's own home.
This post will involve working a rota system across 7 days and some weekend working.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
