We’re looking for someone to join and lead our Hospital Discharge Service located at the Integrated Discharge Hub (IDH) at Royal Stoke Hospital. You’ll provide a coordinated function within the IDH to support safe and timely discharges. Working with partners at the hospital, and within the voluntary and community sectors to support individual needs. Job requirements Line manage a small team. Co-ordinate with the IDH Manager and wider team for a seamless integration and communication process. Collate quarterly monitoring data and attend project review meetings. Lead and support IDH colleagues by identifying patient resources. A flexible approach to covering Co-ordinators role to ensure service demand is met. What we’re looking for: Someone who’s educated to a degree level or qualified with substantial experience. Excellent communication skills. An understanding of what great customer service is. An ability to fulfil contractual commitments for commissioned services. Ability to work under pressure and manage multiple tasks and demands. Idea generation and implementation.
