MAIN DUTIES AND RESPONSIBILITIES: Supporting the family and the team to carry out the complete range of care for the child / young person which can include: provision of personal care, mealtimes, medications administration, gastrostomy care, tracheostomy and ventilator care, emotional care and social/play interaction. Accompanying the child/ young person on trips out of the home to include hospital appointments and/or admissions where required. Contribute to a team approach to patient care in conjunction with all members of the multidisciplinary team. Ensure full understanding of the Support/care Plan and communicate accurately and efficiently how the support/care plan is being delivered. Participate in regular meetings and attend other meetings as requested and required. Attend and comply with all statutory and mandatory training requirements, this includes ensuring E-Learning is maintained and renewed as required. Attendance to all Mandatory Training sessions to maintain competence in line with clinical requirements. Undertake tasks and procedures, in which competency has been assessed and recorded specific to the care environment, including for example, tracheostomy and ventilation training. Behaving in a manner that does not put themselves, children and young people, families, other team members or others in danger or at risk in the work environment. Assist in the maintenance of stock levels, including medicines and PPE and to inform the office/clinical team as and when stock is needed to be reordered. Assist in the cleanliness and tidiness of the home environment and adhere to infection control policy and procedures at all times. Assist the child/young person to ensure they have the opportunity to meet their individual dietary requirements whether orally or enterally fed. Complete legible and accurate daily written reports on all service users in accordance with Yorhealth Policies and Procedures. Become familiar with all organisational policies and procedures and to comply with these at all times. Maintain the confidentiality of information about children, families, staff and company business and meet the requirements of the Data Protection Act (1998) at all times. The postholder must comply with all Information Governance policies at all times. The work of Yorhealth is of a confidential nature and any information gained by the postholder in their role must not be communicated to other persons except where required in the recognised course of duty. Be aware of the responsibilities placed on you under the Health and Safety at Work Act (1974) and any subsequent relevant legislation and always follow these in full including ensuring that they act in line with all agreed procedures at all times in order to maintain a safe environment. Protect supported individuals from abuse and report any concerns immediately to management colleagues. In order to achieve this, you are required to read Yorhealths Safeguarding Policy, participate in regular training and incorporate preventative strategies into your work.
