About the role Fareham and Gosport Wellbeing service is looking to recruit a PCN Wellbeing Advisor. You will be based in the Wellbeing Centre in Gosport and occasional work base from Costal PCN surgeries. You will work solely with patients living in the Coastal PCN area, this role may on occasion include group facilitation in the Coastal PCN area’s. In this role you will hold a caseload of patients offering assessments and 1 to 1 telephone or online sessions to help manage mental health wellbeing. You will attend meetings to enable patients to get the best service for them. You will support people to access groups and activities within our wellbeing centre and the community. Hours: Part-time,15 hours per week (worked over a minimum of 4 days, time to be agreed). Location: Eastleigh Wellbeing Centre (occasionally Winchester). About you You will need to have a good understanding of a mental health issue, as well as experience of working with people with mental health needs, excellent communication skills and a strong understanding of the key difficulties and challenges people face on their recovery journey. You will need good IT skills and be able to navigate different IT systems.
