Key Responsibilities:
1. Deliver highly specialist clinical pharmacist services within the Trust, including directorate level support such as horizon scanning, cost benefit analysis, and assurance reporting.
2. Act as a non-medical prescriber and work in partnership with professionals to reduce medicines-related adverse events.
3. Thematically review existing medication practices and incident reporting to support safer medication practices.
4. Investigate serious medication errors and report findings to the manager.
5. Develop, implement, and audit unit-wide policies and guidelines related to medication.
6. Publish findings and undertake pharmacy practice research to ensure compliance with medicines management guidance.
7. Ensure compliance with Medicines Law and CQC requirements.
Planning and Organisational Duties:
1. Responsible for the provision of Clinical Pharmacy Services to patients.
2. Take professional responsibility for the dispensary in the absence of a senior pharmacist.
3. Prioritise patients based on clinical and pharmaceutical needs and validate patient medication histories.
4. Advise clinicians on required changes to patient medication based on clinical tests and notes.
5. Ensure VTE compliance and appropriate antimicrobial prescribing.
6. Ensure effective re-use of patients' own drugs and destroy unfit medicines following procedures.
7. Assist in the ordering, issuing, and receiving of medication as necessary.
Communications and Key Working Relationships:
1. Communicate effectively with patients, carers, and healthcare staff about medicines.
2. Liaise with pharmacy team members to ensure clinically effective services.
3. Develop effective working relationships with healthcare professionals.
Responsibility for Finance:
1. Assist in delivering cost efficiencies by advising on medicine usage.
2. Handle cash and prescription charges as per trust guidelines.
Responsibility for Human Resources:
1. Organise own work patterns and demonstrate leadership qualities.
2. Supervise and motivate clinical pharmacists and pharmacy staff.
3. Suggest improvements to procedures through line management.
Responsibility for Health & Safety:
1. Fulfill a proactive role towards risk management and report incidents.
2. Advise on risks associated with medicines and ensure compliance with safety regulations.
Responsibility for Teaching:
1. Develop and deliver training packages for staff involved in medicines handling.
2. Provide teaching sessions to healthcare professionals and pre-registration pharmacists.
