Job Summary
The purpose of this role is to provide high quality specialist palliative nursing care, skills, advice and support to patients, carers and those important to the patient. This role will support the delivery of planned and unplanned care across Rennie Grove Peace Hospice.
Main Duties of the Job
* Be competent to work alone with patients and their families in the home environment, under the guidance of the Clinical Nurse Specialist.
* To be professionally and legally accountable for all aspects of own work including the daily management of patients.
* Supporting those patients allocated from day to day in partnership with other members of the multidisciplinary team and wider system partners and under the guidance of the Clinical Nurse Specialist.
* Ability to work autonomously with designated patients alongside a degree of flexibility due to the unpredictable nature of the role.
* Demonstrating and applying a knowledge of the options for achieving control of symptoms in liaison with other health care professionals.
* Reviewing patients when required and discussing required changes in their care with the Clinical Nurse Specialist, colleagues, and other key stakeholders as appropriate.
* Follow Rennie Grove Peace procedures and statutory regulations relating to the management of medicines.
* Maintain record of equipment borrowed and returned.
* Contribute and participate in service reviews and the development, implementation, and evaluation to drive service improvement.

Rennie Grove Peace Hospice Care started formally operating in July 2023, through the merger of two successful organisations, Rennie Grove Hospice Care and Peace Hospice Care. We offer exceptional, specialised and highly valued care, serving our local community of people affected by life-limiting illness or bereavement, across Herts and Bucks.
Job Responsibilities
* Actively participate in the development of the organisations planned and unplanned community Hospice at Home model of care.
* To work as part of the multidisciplinary team to provide a high standard of individualised patient/family care and support.
* To be responsible for those patients allocated daily.
* To ensure that excellent standards of support and clinical practice and care are achieved.
* Deliver specialist palliative nursing care to patients requiring planned and unplanned care. This will include a multi-disciplinary approach for our higher acuity patients with daily virtual and in-person reviews in their own homes.
* To work with the wider Hospice team as well as system partners to support delivery of care and enable patients to remain in their preferred place of care.
* Improve patient outcomes and experience by providing physical and emotional support to patients, coordinating their care services.
* Participate in activities such as audits and case reviews to improve the Hospice at Home service.
* Act as a Rennie Grove Peace Hospice Care ambassador within the community, actively demonstrating our values and positively promoting the specialist care we provide.
* Work across Rennie Grove and Peace Hospice Care geographical area (as required) to support safe and effective service delivery.
Person Specification
Qualifications Essential
* First level Registered Nurse or Paramedic.
Additional Criteria Essential
* -Willing to or working towards of have completed further study in Palliative Care or relevant area.
* -Commitment to follow the organisations education pathway.
* -Evidence of continuous personal and academic development.
* -Proven palliative care experience or related speciality.
* -Proven experience of effective team working.
* -IT literate including a good knowledge of Excel, Word, a clinical IT system.
* -Excellent written and verbal communication skills.
* -Good organisational skills. Able to prioritise clinical duties.
* -Drive and enthusiasm with a positive approach.
* -Ability to work autonomously and as an effective member of the team.
* -Caring and empathetic approach.
* -Commitment to the organisation mission and values.
* -Sensitivity and understanding to the needs of your team and customers, many of whom may have experienced Rennie Grove Peace Hospice Care services or be bereaved.
