An exciting opportunity has arisen at the hospital team that supports service users being discharged from the hospital setting/home first/community hospital beds.
The right candidate will enjoy working in a fast-paced team with a multitude of professionals.
We look at supporting service users using a strength-based approach to promote their wellbeing.

This position is exempt under the Rehabilitation of Offenders Act 1974, and the successful applicant will be subject to a criminal record check from the Disclosure and Barring Service (DBS).
