Herefordshire Council has a fantastic opportunity for a Team Manager to oversee the County and Younger Adults teams for a 3-month contract. This position is pivotal in enhancing adult social care services as outlined by the Care Act 2014. What you will do:
* Lead the County and Younger Adults teams to enhance hospital discharge practice and processes.
* Partner with various stakeholders, including health services, social care, commissioning, and Hoople, to ensure timely and effective discharge.
* Manage complex cases involving learning disabilities, mental health needs, and other challenges.
* Support the transition of young adults under 25, including care leavers, into adulthood with a focus on achieving improved life outcomes.
* Oversee the integration of services to facilitate safe discharges from community and mental health hospitals.
* Drive sustainable change and promote preventative and proactive community services.
* Offer guidance and support to staff, fostering their development and ensuring service excellence.
