What you will do:
1. Provide comprehensive assessment and treatment for a variety of patients in community settings, both in community hospitals and in their own homes.
2. Develop and implement individualized care plans, focusing on patient-centred goals.
3. Collaborate with multidisciplinary teams to ensure holistic care and support for all patients.
4. Offer expert advice and support to patients' families and carers to promote ongoing care and rehabilitation.
5. Maintain up-to-date records and ensure compliance with all HCPC regulations.
Requirements:
1. HCPC registration is essential.
2. Minimum of 2 years' experience as a Physiotherapist or Occupational Therapist.
