We are looking for an energetic person to work with the citizens of Rushcliffe and the staff across the 11 Rushcliffe GP practices, to support them using digital means to share information and get the best from local health services. You will work within a supportive and dynamic team to develop your engagement skills to promote and support digital inclusion across Rushcliffe Primary Care Network.
Main duties of the job
To support patients and the public across Rushcliffe Primary Care Services through digital means. Promoting the use of the internet, Healthcare apps (NHS App) and other technology to help people get a better NHS primary care service. This will involve working with practice staff to make it easier for patients to register & understand how to use online services, as well as educating patients and the population of Rushcliffe. The post holder will focus on reducing health inequalities by increasing digital inclusion for patients and support practices in reducing administrative tasks through empowering patients to take more control of their digital healthcare. The post holder will use social media and websites to promote practice information, with the hope of reducing patient contact with the practice. The post holder will work with practices to improve access to appointments digitally, supporting with implementation of digital tools to improve patient access and care across Rushcliffe.
The post holder will need to travel across the Rushcliffe area, working in practices as well as liaising with other local providers such as the Borough Council and third sector providers to set up outreach clinics in local libraries and health centres. This is a patient, public and staff facing role, using engagement and communication skills to help people who may feel less confident with digital technology become more confident and knowledgeable about services and access.

1. Contribute to the positive promotion of digital health tools, and readily promote and support access of digital health tools available (the NHS App and SystmConnect) to patients & colleagues in the PCN to help people manage their health, care and wellbeing through improved access to services.
2. Creating and utilising social media and online platforms to spread healthcare information to patients across Rushcliffe, improving public knowledge of services available.
3. Work with practices and practice staff to identify, reduce and remove the barriers to digital inclusion for people that experience the greatest health inequalities.
4. Support the facilitation of workshops and task and finish groups. Utilise communication and presentation skills to engage people and present difficult issues clearly to a wide range of audiences.
5. Support the development of stakeholder communications such as newsletters and briefings. As well as provide case studies in relation to the projects impact on our workforce, citizens and volunteers as requested by colleagues.
6. Responsible for managing and coordinating, with support from the PCN, the delivery of the digital inclusion project across the PCN.
7. To support with project schemes and initiatives at planning, development and implementation stages.
8. Responsible for monitoring the progress of the digital inclusion project objectives and ensure that reporting of actions takes place. Then using this information to advise on, initiate and develop services as appropriate.
9. Evaluate the impact of the projects schemes and track and monitor post implementation benefits.
10. Build and maintain networks working effectively with local colleagues to deliver the objectives of the role.
11. Complies with the policies and procedures of the organisation.
12. Support practices in improving their patient access to healthcare using digital tools, cloud-based telephony, and digital solutions (Ardens).
