The Memory and Later Life Service is for anyone who has new or suspected memory problems, including those with a diagnosis of dementia, and people in later life who have anxiety, depression, or psychosis made more complex by physical health needs.
Memory and Later Life services are available throughout Cumbria, covering Carlisle, Eden, and surrounding areas.
We offer assessment and a range of evidence-based interventions in various settings including the person's home, GP surgery, care home, clinic, or hospital, depending on the individual's circumstances.
Main Duties of the Job
The post is for a vacancy within our early memory pathway as a Band 5 Community Nurse. Your main role will be to carry out assessments for individuals following concerns expressed about their memory.
Our team works closely with the person and their family, as well as colleagues in other parts of healthcare, Adult Social Care, and other community and third sector services. Staff within the service include Consultant Psychiatrists, Nurses, Occupational Therapists, Psychologists, Assistant Practitioners, and Support Workers.
We are a supportive team with a variety of experience.
Job Responsibilities
Once we have carried out a holistic initial assessment, we discuss the results with one of our doctors or with our Multi-Disciplinary Team. We will then be able to inform the individual of what we believe to be the cause of their memory problems and if there is a diagnosis. We will explain this to the individual and ensure that they are able to access relevant support, advice, or further information.
Once we have clarified a diagnosis, we will discuss any further assessments, treatment options, or interventions with the individual and their loved ones where appropriate.
Person Specification
Qualifications & Experience
* Experience of working with service users relevant to the post
* Completion of preceptorship programme for newly qualified nurses
* Proven ability to communicate interventions to service users
* Multi-disciplinary team work
* Further relevant post-qualifying training
* Previous experience in a community setting
* Experience of involving service users/carers in evaluation, planning, and monitoring of services
Knowledge and Experience
* Knowledge of relevant contemporary professional theories of mental health care and evidence-based interventions
* Understanding of the statutory issues that impact on service users
