Are you looking for a new challenge? Ancora Medical Practice is recruiting for two Advanced Clinical Practitioners to join our team at our practice based in Scunthorpe, North Lincolnshire.
The successful applicants will deliver a high standard of patient care using autonomous clinical and prescribing skills combined with a broad and in-depth theoretical knowledge base of primary care.
We are looking for experienced Advanced Clinical Practitioners who have experience in prescribing for minor illness, minor injuries, and long-term conditions. The post-holder will demonstrate safe clinical decision-making and expert care for patients within general practice.
The applicant will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients and support the delivery of policy and procedures.

The main duties of the role will include:
* Providing high standards of patient care, embedding clinical policies, guidelines, and standardising clinical governance within the practice.
* Supporting the development and training of advanced clinical practice within primary care.
* Working with the Practice Manager and GP Lead to develop the service.
* Developing a team of Advanced Clinical Practitioners within the organisation.

Job responsibilities
JOB SUMMARY
The post holder will be an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care.
Demonstrate safe clinical decision-making and expert care for patients within the general practice.
Perform as an autonomous practitioner working independently and with other members of the healthcare professionals to assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions, initiating direct referrals as appropriate.
Manage daily surgeries comprising face-to-face consultations, home visits, and telephone consultations.
Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Take a lead responsibility for being the point of contact and coordinating the clinical management of registered patients who reside in local Nursing and Residential homes.
Build effective working relationships with other healthcare professionals and will work within the team of General Practitioners and Nurses to deliver excellent primary health care services within the practice.
Person Specification
Qualifications
* Current professional registration (HPC or NMC)
* Independent Non-Medical Prescribing
* Educated to Degree level
* Qualified or working towards a MSc in a relevant area
* Qualification in Minor Illness/Minor Injuries
* Full understanding of relevant policy, legislation, drivers, and their application to clinical and service area (e.g., Vulnerable Adults, NICE Guidelines, NSF for Older People)
* Advanced/expert understanding of relevant clinical practice, standards, and protocols within identified clinical area
* Demonstrable experience of working in the specific field the post is held
* Evidence of sound post-registration professional practice
* Proven leadership experience, able to demonstrate examples of positive cultural change in service delivery/practice
* Good working knowledge of relevant IT systems to own area of work
* A range of advanced clinical practice skills within a specific area of practice/speciality
