A young lady in Centre of Carmarthen requires a Personal Assistant for 5 hours per week to support and motivate her to declutter and organise her flat. This is essential to support her should she find a suitable property to move to in the future.
Direct Payments Personal assistant (PA) to support the lady to ensure that any mail or correspondence has been dealt with and filed appropriately. She has good speech and language, however needs support with complex information to ensure she has understood what is being communicated. She needs support with written correspondence such as utility bills and benefit forms.
The lady manages her finances independently, but occasionally needs support with any complex issues that arise to do with her finances.
Those supporting her to be fully aware of her Epilepsy Management Plan, and follow this in the event of a non-epileptic attack.
Direct Payments PA to support her with her health, The lady is independent with taking her medication and is independent with initiating and visiting GP, however she requires support to attend to clarify information from GP and for staff to communicate this information back to the relevant professionals in order to ensure that there is no confusion. Those supporting her to feedback any information given by the GP or consultants to CTLD Community Nursing.
Direct Payments PA to support her when she is anxious by distracting her from focusing on her health with positive reinforcement. Those supporting the lady to remind her of what has been advised by health professionals.
Those supporting the lady to remind her to take her boot off when indoors to avoid further damage.
The lady needs prompting to visit other health practitioners such as opticians, dentist and other health appointments.
Direct Payment staff to encourage her to test her Telecare alarms once a month to ensure they are working correctly.
Those supporting the lady to encourage her to re-engage with community groups, volunteering and training. Staff to encourage and motivate Rachel to take part in social and leisure activities, particularly through times of low mood.
Responsibilities:
Supporting the employer with encouragement in gaining independence
Supporting the employer to develop independent living skills
To provide companionship in the employer’s home, including engaging in conversation with them and providing them with social interaction
Encouraging the employer to attend appointments
Ensuring the employer’s safety at all times during your working hours
To carry out any other reasonable tasks that are requested by the employer during your working hours
To ensure compliance with Data Protection Act.

Be responsible for contacting the GP, Social Worker or Local Authority should the employer need medical assistance.

Job Working Environment:

8. Able to develop long-term supportive and consistent relationships with employers.
9. A full, clean driving licence and use of a car is likely to be required. This is determined by the needs of the employer and whether you live within walking distance.
10. You must be prepared to have business use on your vehicle insurance to be able to use the vehicle within work hours
11. Be prepared to undergo training in order to best meet the employers need.
