This role will form a key part in the support of women to maintain a healthy lifestyle during pregnancy and re-establish delivery of the maternal weight management service.
The successful candidates will have a keen interest in supporting people with behaviour change and facilitate positive outcomes in pregnancy and birth, helping to provide the foundations for healthier families.
The post holder will provide high quality interventions to meet the needs of women, offering practical support, through delivery of nutrition courses, walking groups or safe exercise programmes.
The post holder will work under the supervision of a qualified health care professional with the ability to work independently, demonstrating an awareness of the limits of professional practice and knowledge to recognise when to seek appropriate support and advice.
This role is essential in the delivery of the Healthy Weight: Healthy Wales strategy within maternity services, working towards an increase in the number of adults and children who are a healthy weight as part of building a "Healthier Gwent".
Main duties of the job
Based within the Weight Management Service at St Cadocs Hospital you will become an integral member of the Healthy Pregnancy team to independently deliver programmes to support eating well and being active in the antenatal and postnatal period.
This role works across the Aneurin Bevan University Health Board (ABUHB) area, alongside hospital and community midwifery departments. Travel to different sites to deliver interventions is an essential part of this role and will include both face to face and elements of home working.
As Assistant practitioner, you will have your own caseload of clients, reporting to a lead health professional for supervision and support to deliver weight management interventions.
This role will be responsible for:
1. Managing referrals
2. Assessing, advising and monitoring individuals
3. Using behaviour change and motivational interviewing techniques.
4. Delivering group intervention such as the Foodwise in pregnancy programme and safe exercise programmes
5. Liaising with a range of partners and organisations to support on-going lifestyle change post-natally
6. Health promotion activities
7. Ensuring outcome data is collected
8. Following direction of the supervising dietitian/public health midwife.

Qualifications
* Advanced apprenticeship/foundation degree in health related topic
* Nutrition related qualification to NVQ level 3 or equivalent
* Agored Cymru Level 2 Community Food and Nutrition Skills
* Teaching Qualification eg PGCE (PCET)
Experience
* A good understanding of nutrition, physical activity and weight management
* Experience of applying knowledge to Public Health needs in complex community environments
* Evidence of CPD e.g. portfolio
* Experience and underpinning knowledge of behaviour change techniques and their usage
* Knowledge of educational limitations in others including potential barriers to meeting nutritional requirements for this type of client.
* Knowledge of impact of social determinants of health and resulting impact on meeting wellbeing goals.
* Experience of working in a healthcare role such as NHS, Social Care or community setting
