We are looking to recruit to thepost of Nursing Associate, to work within our Primary Care Network healthcare team.
Wentworth 1 PCN is a partnershipof 7 GP practices, Magna Group, Crown Street Surgery, High Street Surgery,Parkgate Medical Centre, Shakespeare Road Medical Centre, York Road Surgery andRawmarsh Health Centre. We have a patient population of over 52,000 people.
Main duties of the job
The postholder will work with adiverse range of people from different cultural and social backgrounds. Theability to work confidently and effectively in a varied, and sometimeschallenging environment is essential.
The successful candidate willhave excellent interpersonal and communication skills, and be organised,patient and empathetic. They will have experience of working in health, socialcare or other support roles including direct contact with people, families orcarers.
Continuing learning is essentialwithin this role and you will be offered opportunities to continue yourprofessional development.

This new way of working brings together partners from across the health, social care and voluntary sector to design and deliver innovation in services better placed to meet the needs of our communities, ensure better health outcomes for patients and utilise resources in a more efficient way. This includes the creation of additional roles in our practices including Clinical Pharmacists, Social Prescribers and Care Coordinators.

Job responsibilities
1. To work as part of the PCN's Multidisciplinary team ) to provide and monitor care, under direct or indirect supervision.
2. Improve safety and quality of care at every opportunity
3. Contribute to the delivery of integrated care
4. Support registered Practice Nurses.
5. Develop relationships across the MDT to support integration of the role across health and social care including primary care, secondary care, and mental health
6. Perform and record clinical observations such as blood pressure, temperature, respirations, and pulse.
7. After undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role
8. Promote health and well-being to all patients, for example undertaking the NHS health check
9. care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities
10. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
11. maintain accurate and contemporaneous patient health records
12. enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.
