Main duties and responsibilities Patient Care: To provide patients and staff with specialist advice to deliver best practice stoma care To continue to develop, improve, and promote the stoma care service ensuring the deliveryof high quality, cost effective care. To develop and share expertise within the multidisciplinary team, advocating for theservice and the patients. To develop the requisite skills to act as a resource in the care of patients, ensuring an optimum level of service at all times. To work within UHNM hospital trust (Stoke and County), liaising with relevant professionals to ensure an effective and seamless service. To effectively provide support for patients using diverse forms of communication. To build up a skill set to embed patient and public involvement within the sphere of specialist stoma care nursing practice. To work as a patient advocate, to help negotiate the patient journey and ensure optimum care is provided. To develop links with other local and national organisations providing support and information. To adhere to infection control guidelines and procedures. To act as a positive role model through the demonstration of excellent communication and clinical skills. With support develop the skills to analyse and respond to complex clinical situations eventually working towards practicing as an autonomous specialist in stoma care. Provide holistic assessment at various stages of the patient pathway ensuring patient is seen and cared for as a whole individual. Understand the specific psychological challenge related to adaption that ostomates (stoma care patients) face, and respond appropriately signposting if necessary. Willingness to understand the range of product available to patients, select appropriately and act as a gatekeeper and advocate in this regards. Familiarise self with local policy, national guidelines, and in particular ASCN competencyframework. Communication and Relationship skills: Demonstrate excellent communication skills to include a non-judgemental attitude to all ages,cultures, sexualities and genders, recognising their unspoken needs and exercisingcounselling skills as appropriate. Contribute towards sharing good practice within the multidisciplinary team, and becomeconfident in educating colleagues. Demonstrate a professional pride in your department, colleagues, and the care you provide. Work cohesively with all members of the multidisciplinary team and promote effective liaisonwith the team in ensuring that the very best services to patients are provided at all times. Communicate effectively within the multidisciplinary team; participate in motivating andsupporting team members. Maintain confidentiality regarding patients and carers. Give written and verbal advice/teaching to patients and carers. Maintain and foster relationships within the wider stoma care community, including local andnational networks and patient advocate groups. Professional Development and Education Responsibilities: To participate and develop the planning and delivery of formal and informal educationprogrammes for patients, carers and all staff. To participate in education forums locally. To participate in individual performance review annually. To ensure that professional knowledge and practice is constantly updated and shared. Takes personal responsibility for life-long learning and personal development through clinical supervision, appraisal and Knowledge and Skills Framework. Leadership and Management: Planning the provision of day to day departmental tasks With support manage the devised pathway based on flexible principles, which prioritises the careneeds of patients. Have a working awareness of the UHNM NHS Trusts complaints procedure, escalating appropriately to your line manager. Maintain accurate and concise records that produce statistical data for the purpose of evaluationand development of the service. To keep accurate and up to date patient records. Actively engage with appropriate agencies to contribute expertise and experience In partnership with the line manager develop the skills to provide specialist nursing advice andsupport to patients including their families. To assist in the development of evidence based policies and procedures relating to the speciality To assist in the development of quality initiatives such as audit, evidence based practice and riskmanagement within a culture of continuous quality improvements. To practice in accordance with current policies and procedures of UHNM NHS Trust To act as a change agent, using appropriate communication skill that will motivate and reassurestaff through negotiation and training, to facilitate the change process. The post holder may be required to order supplies as advised and/or safely use equipment To record all patient related work to ensure this is commissioned correctly
