Job summary The Learning Disabilities Health Facilitation Team supports people with Learning Disabilities to access health care within community and acute hospital settings. We have an opportunity for an experienced clinician to join our team for a one year fixed contract. The successful candidate will be enthusiastic, proactive and creative with experience or interest in working with people with Learning Disabilities. We see patients across West Sussex, the successful applicant would be based at Littlehampton Health Centre and cover the Worthing, Adur and Arun GP services. We work to promote best practice, and ensure the delivery of high-quality care for people with Learning Disabilities. We work closely with the relevant MDTs, including GPs, Consultants, Special Care Dentistry and community services. We advise and support complex best interest decision making, reasonable adjustments and care planning. We deliver a Learning Disability Champion programme, and the successful candidate would be part of the planning and delivery of sessions and support. We are an inclusive service, and won the SCFT Inclusive Champion award in 2021. We were nominated again in 2022 and continue to seek opportunities to develop our service. Main duties of the job The role of the Health Facilitator will be to improve and facilitate access to care in primary services, through supporting the GP practices and community services with health issues and the needs of people with learning disabilities. The post holder will also cover some work as an acute liaison nurse to allow smooth transition to and from hospital services.
