An exciting opportunity has arisen for an Enhanced Clinical Practitioner to join the Advanced Practice team within the Purbeck Hub delivering care to the residents of Purbeck. We welcome applications from allied health professionals and nurses.
As part of the Network, you will be joining a multi-disciplinary service to provide urgent community response and admission avoidance to our patients whilst working with the team on service delivery, triaging and prioritising visits to achieve our overall objectives.
You will be based within the Purbeck Hub, Wareham along with an excellent team of Practitioners, who are supported by a GP, a hospital consultant and long-term condition HCA's.
You will provide clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and decision-making skills. Your role will be community based supporting people in their own homes.
This is a fixed term contract for 1 year covering maternity leave. Hours are part time at 22.5 per week between 8.30am and 5pm; as the service develops shifts may cover 8-8pm with possible weekends.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Main Duties and Responsibilities:
1. To provide high-quality person-centred care which always considers people’s safety, privacy and dignity using enhanced clinical knowledge that is evidence based.
2. To undertake independent assessment and management of service users to incorporate pharmacological considerations, where appropriate making complex clinical decisions regarding service user management and clinical outcomes.
3. To provide enhanced clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and enhanced decision-making skills.
4. To manage change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research, and education. To promote and demonstrate best practice by integrating evidence into practice.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
