Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for an Adult Social Worker within the Hospital Progress to Discharge Team to work full time based in Hull.
The salary for this permanent Adult Social Worker job is up to £36,648 per annum.
Main duties:
* To promote and safeguard the welfare of adults at risk, families, and their carers, within the ‘making safeguarding personal’ principles.
* Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose. Ensure people are given correct financial advice and information about charging processes of the council.
* Work in partnership with Occupational Therapy and Physiotherapy, utilizing their expertise in the assessment and review process. Assess, identify, and source low-level OT equipment that could support an individual to live independently or as part of wider packages of care.
* Engage in critical reflection and constructive challenge while working with colleagues, other agencies, and vulnerable adults and families.
* Actively participate in team meetings/practice forums and other relevant meetings, ensuring focused and purposeful social work is undertaken with all cases.
* Collaborate with colleagues in the team to ensure communications are clear and aligned with people’s needs. Demonstrate effective use of evidence-based social work practice, ensuring effective communication with individuals who have specific communication needs including learning disabilities, dementia, those who lack capacity, and individuals with sensory impairments.
* Engage in and advise on the professional decision-making process with adults at risk, families, and their carers, and colleagues regarding complex/high-risk cases.
* Prepare for and attend court as required in relation to specific cases where the engagement of legal processes is necessary.
* Actively support and engage in practice development groups and provide information/professional opinion as required, e.g., for policy amendments and newly developed ways of working.
* Take a leadership role in managing the daily activities of the team, including allocation and duty work.
Requirements of this Adult Social Worker role:
* A professional Social Work qualification or equivalent.
* Understanding of relevant legislation.
* Registered or eligible for registration.
