Job Title: Social Worker – Community Mental Health Older People
Location: Vale of Glamorgan
Rate: £33.52 per hour
An exciting job role has recently become available for a Social Worker. They are looking for a new member to join the Community Mental Health Service for Older People in Vale of Glamorgan.
What will your responsibilities be?
You will conduct assessments, plan and commission services, and work in partnership with citizens and carers to achieve personal outcomes. You will also manage a specific caseload, coordinate resources, and maintain accurate electronic case files. The team offers specialist secondary mental health care for adults living with dementia and people over 65 years with mental health problems. The team works alongside specialist NHS services to support people to live well and safely in their own communities through the provision of recovery-focused care and treatment.

Qualifications and Experiences:
To be successful in this role you must have Social Care Wales registration, experience of working with adults with long-term conditions, be an experienced qualified social worker, have experience of safeguarding, and experience of working with adults and self-neglect.
