The Adult social work/Occupational Therapy teams are responsible for the Short- & Long-Term delivery of person centred support and an asset-based care planning/ equipment provision for Adults over the age of 18 years. Halton Borough council’s expectations are high, and you will need a strong commitment to providing first-class person-centred care to Halton residents and support to their carers. You will be passionate about working with a range of service user groups -older people, adults with Mental Health difficulties, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence. The Mental Health social work team works with adults, across 2 teams, 18-65 years & Older People Mental Health Teams, who meet the criteria in line with the Care Act and require support in relation to their mental health. We work closely with our Mental Health Trust colleagues to provide a responsive, holistic and recovery focussed response to adults with complex mental health needs. The teams work closely with The LLAMS team, Home Improvement Agency, Joint Equipment provision, Independent Living Centre, Continuing Health Care Nurses, CPN’s, Specialist Learning Disability Nursing Service, Neuro-Rehabilitation Team, GP Practices including District Nurse services, Community Physiotherapy and Occupational Therapy. The teams work in partnership with other teams and departments internal to Halton Borough Council including Children and Family Services, and the Positive Behaviour Support Service. You will have experience in completing assessments under the Care Act, an understanding of the Mental Capacity Act and the completion of Continuing health care assessments. You will have a recognised professional qualification Social Work Degree, Occupational Therapist Degree, MA, Dip SW or equivalent, good interpersonal and communication skills a flexible approach and a willingness to continually improve your practice; in addition, you must hold a full driving licence.
