Social Care Worker Worcester 22/04/2025 – 2 months initially Monday to Friday, 9am – 5.30pm, 37 hrs/week Ensure that the outcome of all work with adults in need of care and support, and with their carers, is to promote their wellbeing Carry out and review Needs Assessments & Care and Support Plans for individuals and their carers Engaging with people to understand the reason for their contact and their presenting needs and to consider options for resolving these at an early stage Ensure opportunities for reablement and independent living are always considered and explored, enhancing individual and family resilience Work with the individual in their local environment supporting the individual to build support within their local community Ensure information and advice is provided Work closely with primary care/community services/hospital teams/voluntary community services Keep and maintain records of work Attend meetings and reviews where necessary Requirements A professional qualification relevant to the post such as Health and Social Care Experience working with Adults with complex needs Experience in assessments and support planning Adult DBS registered to the update service Recruitment is done in line with safe recruitment practices. 4Recruitment Services in an equal opportunities employer.
