Senior Community Nurse for Children with Disabilities
An exciting opportunity has arisen within the Conwy & Denbighshire Children's Learning Disability Nursing Service for an experienced, enthusiastic NMC registered nurse.
To work within a multi-disciplinary team closely with staff from other agencies to promote and maintain high standards of care for children and adolescents with learning disabilities, developmental delay, and complex needs. This will involve assessing developmental and associated health needs, planning, implementing, and evaluating appropriate programmes of care. Where required, more specialist assessment will be undertaken under the indirect clinical supervision of a Senior Nurse or Team Leader. Such work will be underpinned by ordinary life principles and evidence-based practice. The post holder will be expected to work with children, their families, carers, staff teams, and other professionals in a variety of community settings with ongoing support. In addition, the post holder will assist with service development activities within the team.
Main Duties of the Job
* To take responsibility for allocated referrals from source to discharge in line with agreed protocols and documentation, including assisting Clinical Psychologists in the diagnosis of Learning Disability and assessing eligibility. This may involve interpreting complex cognitive assessment information or conducting assessments indicating levels of functioning.
* To conduct initial 'choice' appointments (or equivalent) with carers to agree on the suitability of the service to meet the child's needs and agree on outcome-focused goals to work towards.
* To undertake an initial assessment of physical, psychological, and social well-being in order to accurately identify the child's needs and associated health needs.
* To carry out this assessment utilizing evidence-based tools and, where appropriate, to provide more detailed specialist assessment of specific areas of difficulty such as behavioral difficulties using functional assessment tools, rating scales, and collection and analysis of data such as ABC records. The post holder will then collate the information into a cohesive formulation of the problems to enable the development of plans/programmes to address issues.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
