Our client provides excellent specialist care for people with complex rehabilitation care needs - from slow-stream rehabilitation through to end-of-life care. They specialise in assisting people with long term conditions and co-morbidities, progressive conditions, end of life care, trauma, re-enablement and rehabilitation, promoting quality of life, choice, dignity, and independence wherever possible.
Job Role
1. To provide high quality patient-centred care through assessment, development and delivery of individual interventions and sessions. To take responsibility for own caseload, within a defined area of service or for a particular patient type.
2. To contribute to support sourcing and review of suitable, patient-specific equipment, including but not limited to orthoses, assistive devices, wheelchairs, comfy chairs, and EATS equipment.
3. To support the review of patient moving and handling, supervision of junior staff, and working alongside a multi-disciplinary team including occupational therapists, physiotherapists, speech and language therapists, therapy clerk, nurses, care staff, and therapy assistants.
4. Be able to assess patients with diverse presentations and complex physical and psychological conditions, formulate and deliver individual treatment plans in accordance with their assessments.
5. Moving and handling assessments, initial and ongoing.
6. Have excellent communication skills, written and verbal/non-verbal.
7. Be able to take responsibility to work autonomously providing holistic treatment programmes.
8. Appropriate equipment assessment and provision within the home.
9. Instruct therapy team assistant/care staff or relevant members of the MDT in the delivery of treatment programmes.
10. Maintain records of treatment and interventions in keeping with COT standards of practice and Pathfinders Policies.
11. Be professionally and legally responsible for all aspects of their own work in accordance with HCPC guidelines and COT regulations and standards.
12. Maintain their own level of competency through CPD and personal development planning.
13. Attend in-house training and provide formal/informal training to unqualified staff as required.
14. Direct other staff in their roles supporting the rehabilitation and physical management of patients, i.e., ADLs, manual handling etc.
15. Maintain their own safety and that of work colleagues and service users at all times.
16. Support risk assessments and reporting where necessary.
17. Support community re-integration including home visits, access visits, outings, and activities of daily living.
Requirements:
1. Experience of equipment assessment
2. Experience with moving and handling assessments
3. Current COT and HCPC registration
4. Knowledge of Health and Safety, particularly Moving and Handling techniques, Fire Safety, and COSHH.
5. Knowledge and Understanding of Care Standards Act 2000 and other relevant legislation.
6. Display good interpersonal skills and ability to instruct and demonstrate care activities.
7. Demonstrate good organisational skills.
8. Exhibit a caring disposition towards service users and acknowledges the need for varying approaches to meet individual needs.
