The therapy team at Ascot House Intermediate Care Unit are looking to recruit a part time (22.5 hours) Band 3 rehab assistant who has a desire to deliver therapy interventions to older adults who require a period of short term Intermediate Care to return to their home safely.
Ascot House is a therapy led intermediate care unit, which is commissioned and delivered via integration between Trafford Local Care Organisation and Trafford Council.
This is an exciting opportunity to join the well established therapy team at Ascot House who work in partnership with Trafford Council to provide a patient focused Intermediate Care service.
The team are based within Ascot House and are led by a Clinical Lead Physiotherapist and an Occupational Therapist Senior Practitioner.
Ascot House is based in Sale and the current working pattern is Monday to Friday (22.5 hours flexible over 3 days).
Key attributes for potential candidates include:
Experience within older adult therapy services
Experience of intermediate care services
Knowledge and experience with equipment and adaptations
Adaptable to change
Experience with exercise programmes
Well organised
Person centered
Equipment and moving and handling skills
Experience with clinical systems
Awareness of clinical documentation
Awareness of the wider MDT
The post holder will support the therapy team with the rehabilitation and reablement of patients within Ascot House with guidance and support from the allocated Occupational Therapist and Physiotherapist.
The role includes but is not limited to:
Practicing mobility and transfers with patients
Delivering exercise programmes as prescribed by the allocated physiotherapist
Accompanying clinicians on environmental visits, home visits and discharge visits
Supporting the team by picking up and delivering equipment
Providing verbal handovers on the progression of the patients to help the allocated therapists make decisions regarding discharge
Being part of a team who deliver patient centered goals and outcomes
The team is committed to integrated working and you will have the opportunity to work alongside physiotherapists, occupational therapists, nurses, medics, a pharmacy technician and the social care team.
As the caseload is often demanding, candidates should be well organised and self- motivating. Above all you must be able to function well as part of a supportive team.
