The Central and East Locality in Exeter City is a community nursing hub covering 11 GP Practices in Exeter City and some of the surrounding areas. We are based at Whipton Hospital, Exeter.

Job responsibilities
You will be responsible for working clinically to deliver care to patients in their own homes and in a clinic environment. You will be expected to provide holistic nursing assessment and nursing care, advice and information to patients and carers within the home environment or other appropriate care settings. We work closely as a wider team to prevent hospital admission where able to facilitate hospital discharge for patients to be cared for within their own home environment. We strive to support high quality patient care in their home or the most appropriate environment enabling them to maximise their independence and optimise their quality of life.
Experience in community nursing is desirable for these posts but not essential as full training will be provided and the post holder will be supported to develop in the role. There will also be many opportunities to develop and progress your career in the community as community nursing enables a wide range of clinical skills to be used to support some of our most complex patients within their own home.
You will need to have a full driving licence and have access to a car, with business insurance to be able to fully undertake this role. Lone working is expected within this role after a period of induction with ongoing support from our experienced coordinators and triage facilitator.
Working Pattern: 08:30 to 16:30. 7 days per week.
