Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people. This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them. The service has an intensive support element within it that offers extended hours and weekend interventions to people who are seen by the team.
