Job summary This is an exciting opportunity to work as a member of the NHS Lincolnshire Integrated Care Board (ICB) All Age Continuing Healthcare (AACC) Team. The Palliative and End of Life (PEoL) Case Manager will ensure that the care needs of individuals in receipt of fast-track funding are identified and met in accordance with the National Framework 2022 (Revised). You will be working with the Quality Team and ensuring FT individuals are agreed promptly for funding and reviewed to ensure the package of care is meeeting their needs. We would expect you to have a good knowledge of continuing care, although support and training will be given where needed. You will need to be aRegistered Nurse RGN, RMN or RNLD or Allied Health Professional with qualification, have knowledge of the CHC framework and its application, knowledge of NHS and social services guidance Main duties of the job Case managing individuals in receipt of fast-track funding, ensuring timely reviews are conducted and ascertaining care packages continue to be effectively commissioned and are cost effective.
