This would include visits to families, residential homes, and day centres to visit individuals on the caseloads of the team. It could also include responding to telephone enquiries, requesting patient records, and attending Multi-Disciplinary Team (MDT) meetings. Main duties of the job Under the supervision of the CHC Care Manager: To visit individuals in a variety of care settings as well as within the individuals own home. Communicates with individuals who have barriers to understanding and acceptance. To work alongside individuals and their family representative/s ensuring that the support that is in place is meeting the individuals needs. Develop close working links with partner agencies, promoting joint working in a co-ordinated manner. Contribute and support to CHC senior nurse to enable completion of relevant assessments or reviews. Involve individuals in care decisions and gain consent where required. To be competent in the use of SystmOne for management of individual records and information and ensure that each contact with a individual or their family is documented accurately. To report back to the Senior Nurse or another team members any concerns after each visit. To update all the relevant personalised documentation under the guidance of the case load holder senior nurse so that resource/funding decisions can be made.
