Tameside and Glossop Integrated Care NHS Foundation Trust are looking to recruit an experienced Band 7 Rheumatology Specialist Nurse on a 9 month fixed term contract to help cover maternity leave within the service. The post holder will work as an expert practitioner in Rheumatology Nursing, working at an advanced level with the responsibility for a patient case load. Whilst working autonomously the post holder will be expected to work together with Consultants and Nursing colleagues to supportintegrated pathways for the service. The post holder will also work independently in Primary Care post holder will ensure that evidence based practice in inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area as appropriate. The post holder will support the delivery of excellent patient care and experience and the delivery of departmental and Trust objectives. The post holder will ensure that theRheumatology Service continues to work in a safe, clinically effective and patient focused post holder is expected to always act in a manner consistent with the NMC Code of Professional Conduct, carrying out his/her duties in accordance with Trust policies and procedures.
Main duties of the job
To provide and receive routine information to Consultants, Senior Managers and Nursing colleagues relating to Rheumatology. This role will also work alongside and liaise with local GP's for Rheumatology.
To work alongside both internal and external colleagues to ensure the effective delivery of the Rheumatology service.
To plan/organize and maintain a caseload of patients - Including the screening, treatment and education of patients through nurse lead clinics and helpline.
To work to the level of a Specialist Nurse Practitioner providing assessment, diagnosis and treatment of patients with Rheumatological carry out specific procures, such as the Joint Injection Clinic, within the nurse-led service.
Undertake Primary Care sessions as an independent Nurse work collaboratively with colleagues, both nursing and medical, in Paediatrics, Dietetics and Psychology in the treatment of paediatric patients.
To administer appropriate medications using the Trust's Policy & procedures.
To act as a patient advocate on a regular basis.
