We currently have a vacancy within our service and are looking for a highly motivated and enthusiastic professional to join our supportive and dynamic team.
To lead and oversee the day-to-day management and co-ordination of the Continuing Healthcare Learning Disability workstream within the Complex Cases Team and the process for undertaking NHS Continuing Healthcare Assessments, applying the Department of Health's National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
To provide specialist advice and undertake clinical assessments as required with respect to the process for NHS CHC Assessment/Review for NHS Continuing Healthcare, to clients, health and social care professionals and other agencies, within the relevant and specific team, with the ability for cross cover across the directorate as and when required.
Main duties of the job
We are looking for a registered professional who is experienced in working with people who have Learning Disabilities, complex physical and mental health needs.
NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital but who have complex ongoing healthcare needs.
The Complex Cases Team is responsible for the decision-making and commissioning processes, working closely with the Local Authorities throughout Cambridgeshire & Peterborough.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a home-based role, and post holders will be expected to be able to travel across Cambridgeshire and Peterborough.
You will be joining a fast-paced team that employs the use of holistic nursing, inquisitive thinking and a focus on high quality. The nature of the role is autonomous and challenging but with the benefit of supportive colleagues.

Person Specification
Qualifications
* Registered Learning Disability Nurse - Valid NMC.
* ENB 998 Teaching, Assessing and Supervision or equivalent.
Skills, Ability & Knowledge
* Detailed knowledge of National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and its implementation.
* Highly developed knowledge and understanding of NHS and Social Care Services structures and Independent Sector Organisations.
* Fully conversant with the holistic client process and need to assess, plan, implement and evaluate the needs of clients with particular emphasis on in-depth knowledge and wide experience of specific clinical interventions available to support clients with specialist and complex health needs.
* Highly developed knowledge and experience of staff recruitment and selection processes.
* Excellent written and verbal communication skills.
* Proficient in the use of IT systems.
* Excellent presentation skills.
* Ability to Chair meetings.
* Ability to relate to clients, and all levels of multidisciplinary/multi-agency personnel confidently and efficiently.

Experience
* Substantial experience within NHS Continuing Healthcare.
* Leadership experience of managing a team.
* A wide knowledge, understanding and experience of conditions such as Acquired Brain Injury, Neurological Care, Palliative Care, Medical Care, Surgical Care and Long-Term Conditions.
* Experience of working in partnership between health and social care.
* Experience of delivering training programmes to groups.
* Experience of change through partnership working.
* Experience of managing or planning specialist packages of care which are tailored to the needs of individual clients.
* Experience of Community Nursing.
* Experience of Palliative Care.
* Demonstrable experience of working across relevant client pathways.
* Experience and awareness of budgetary issues ensuring concordance.
* Experience of involvement in Continuing Care Multi-Disciplinary Team Meetings.
