We are looking for highly motivated and autonomous nurses, registered with NMC to be part of the growing UCR multi-disciplinary team which will take forward the 2 Hour UCR for Lincolnshire. This service provides holistic assessments, treatment and support to patients in their usual place of residence who are experiencing a health or social care crisis who might otherwise be admitted to hospital or require ambulance call out. You will be part of a service in which care is provided by a multi-skilled team including ACPs, nurses, paramedics, occupational therapists, physiotherapists, assistant practitioners and therapy assistants.
This is an integrated health and social care team that visits patients in their own homes or care homes and assesses the patient’s safety to remain out of the acute environment. Patients are aged over 18 and are likely to present with frailty and falls or health deterioration. The team ensures the patient is supported out of hospital for a minimum 48-hour period and ensure transferred to appropriate services on discharge.
Main duties of the job
As a registered nurse in the UCR service you will be expected to work as an accountable and autonomous practitioner in the assessment, planning, delivery and evaluation of safe and effective person-centred nursing care, with particular focus on assessment of need, coordination of services, and discharge planning that will enable individuals to achieve either their optimum physical, psychological, social and emotional wellbeing.
You will also be part of our falls response service which aims to support patients who have fallen at home and provide a falls pick up response followed by a holistic assessment of the individual's needs to support them to remain safely at home. This will also involve identifying an urgent need and initiate an appropriate response, giving the appropriate first aid actions as necessary within own capabilities.
This role will entail holistically reviewing patients. The role will involve phlebotomy, education of patients/relatives, recording of observations, recording of blood sugars, administration of medication (includes involvement with palliative emergencies), urgent catheter care, liaising with key players in the MDT to ensure the best clinical outcome for the patient, including escalating any concerns to the team leader at the earliest opportunity.

Detailed job description and main responsibilities
As this is a community post, you must be able to travel to patients' homes across the county. Due to the rurality of Lincolnshire, some areas are not well covered by public transport, therefore having access to transport is essential. A full UK driving licence is essential for this post.
As you will be working with patients in their own environment, there will be times when you will be exposed to unpredictable situations. This may include unsanitary conditions, bodily fluids and/or distressing situations/scenarios.
The service works through the hours of 8:00am to 20:00pm 7 days a week, 365 days a year.
An induction programme for all new staff will be included. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential.
