As a member of the organisation, you will have direct contact with children, and young people (CYP), and their families and will be instrumental in ensuring that plans for individual patients, and information that the organisation wishes to share with its patients, are enabled and enacted. The post holder will have excellent communication skills, to ensure that every contact counts, through a range of media (telephone, face-to-face, IT) and will demonstrate excellent patient care through commitment to follow through of established plans to improve health outcomes.
• Main responsibilities will be to support families and other professionals to access and understand the referral process
• To gather information for families, children and young people to support the clinical team
• To signpost and ensure access to needs led support and advice

• To deliver and coordinate the navigator process in line with current service provision
• Maintain awareness of clinical and professional developments and research where it is appropriate to do so, in order to enhance patient care
• Identify, initiate and/or participate in audit and evaluation activities to enhance service delivery
• Maintains accurate records electronically in a professional manner and in accordance with Trust policies, guidance and GDPR regulations
• Be responsible for your own development, applying to undertake relevant training when available
• Highlight risks to Clinical Leads to place on the Team Risk Register
• Comment on, contribute to and interpret professional policies and procedures that are relevant and pivotal to the service
• Communicate effectively complex and sensitive information with patients, their families and heath professionals both within and external to the organisation, adjusting to the level of communication to suit the situation
