The Forensic Low Secure & Learning Disability Forensic Inpatient service is offering an opportunity for a Band 6 Team Leader at the Brookfield Centre.
Brookfield Centre is a 13 bed rehabilitation inpatient unit catering for men from the age of 18. We provide mental health support for adults with learning disabilities (LD). We specialise in the assessment and treatment of men with a learning disability whose offending behaviour and complex mental health needs require care in a locked rehabilitation setting.
The ward consists of a multi-disciplinary team of health care assistants, nurses, occupational therapists, psychologists, speech and language therapists, doctors and consultants.
Patients are offered therapeutic activities by the occupational therapy team and their care is reviewed regularly during ward round and other multi-disciplinary team meetings.
Main duties of the job
Under the overall direction of the Clinical Nurse Manager, maintain high standards of patient care and ward performance. Be professionally responsible for the assessment of care needs and the development, implementation and evaluation of a multidisciplinary programme of care. Support the Clinical Nurse Manager and the multidisciplinary team in the leadership of the nursing team, patient care and ward. Manage the ward in the absence of the Clinical Nurse Manager.
Job responsibilities

2. Provide clinical and professional advice, leadership and support to a multidisciplinary staff group, patients and their families.
3. Attend formal meetings i.e. safeguarding, governance on behalf of the Clinical Nurse Manager when required.
4. Understand and appreciate the need to adapt when there are barriers to communication.
5. Take account of cultural issues in communication with everyone, including work colleagues.
6. Understand what translation and advocacy services are available and follow correct procedures.
7. Ensure records of self and junior colleagues are up to date, accurate and consistent with Trust policy.
8. Establish and maintain accurate, complete and up to date care plans in line with Trust policies and procedures.
9. Effectively use Trust electronic tools and IT systems.
10. Competent to implement strategies which facilitate adherence to treatment.
