The North West PCN Dietician will work within a practicesetting and with the PCN and Practice Team to deliver high quality expert assessment, management and education for patients requiring specialist dietetic and nutrition advice for early intervention of weight management.

The role will be working across various community settings and the GP practice members of the North West PCN.
Main duties of the job
Main duties of the job include:
* provide specialist nutrition and diet advice to patients, their carers, and healthcare professionals through treatment, education plans, and prescriptions
* educate patients with chronic diseases on how they can improve their health and prevent disease by adopting healthier eating and drinking habits
* provide dietary support to patients of all ages (from early-life to end-of-life care) in a variety of settings including nurseries, patient homes, GP surgeries and care homes
* work as part of a multi-disciplinary team to gain patient's cooperation and understanding i following recommended dietary treatment

Job responsibilities
The main responsibilities are set out in the job description and include:
* develop, implement and evaluate a seamless nutrition support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working
* work with clinicians, multi-disciplinary team colleagues and external agencies to ensure the smooth transition of patients discharged from hospital back into primary care, so that they can continue their diet plan
* make recommendations to PCN staff regarding changes to medications for the nutritional management of patients, based on interpretation of biochemical, physiological, and dietary requirements
* implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training
* undertake a range of administrative tasks such as ensuring stock levels are maintained and securely stored, equipment is kept in good working order and updating patient records
* ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives
* undertake and support group or joint consultations as required
Person Specification
Qualifications and Knowledge
* BSc or pre-reg MSc in Dietetics under a training programme approved by the British Dietetic Association (BDA)
* Evidence of registration with the Health and Care Professionals Council (HCPC)
* Able to operate at an advanced level of practice
* Membership of the British Dietetic Association
* Membership of specialist interest groups
Experience
* Understanding of professional ethics and application in practice
* Experience in NHS or Primary Care
* Experience running joint consultations
Skills and Competencies
* Ability to manage a busy caseload
* Ability to recognise and seize opportunities for change and development within the service
* Support patients to set their own goals and actions as part of early intervention
* Person centred approach
