To be part of the practice team and support the delivery of Herschel Medical Centre and CSN PCNs ambitions to the highest quality and safety. Contribute to better management of patients so that they are kept out of the hospital and feel supported. Contribute to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues To support patients recently discharged from the hospital by ensuring they understand their medication, any changes that have happened recently and have a timely supply. Ensure that the practice repeat prescribing policy is regularly reviewed and that it is being adhered to with the aim of reducing waste and improving safety. To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing. Working closely with GPs to resolve day-to-day issues related to the medication. To act as a point of contact in the practice for all medication and prescription-related queries from patients and health care professionals. The role of pharmacist will include but not limited to: Structured Medication Reviews (SMR) Long-term conditions (LTCs) management Care Home- Housebound Medication Reviews Complex Polypharmacy management Elderly and frail patients care Quality Improvement Prescribing Indicators (QOF) Specialist expertise of medicines Lead on person-centred Medicines Optimisation Lead on local Anti-Microbial Stewardship (AMR) within the PCN QOF contribution and Enhanced Services Work with Pharmacy teams across Primary Care, Community and Hospital as part of the wider healthcare team to improve patient outcomes To manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions. Participate and deliver education and training to practice staff of medication-related topics. To present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines. To produce patient information leaflets and posters and run medicine awareness projects throughout the year. To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacists.
