Job summary The post holder will work as part of the wider specialist palliative care team and be responsible for providing specialist/complex advice and support to all members of the multi-professional team in the care of patients with a life limiting disease who are experiencing symptoms that require specialist palliative care. Provide an effective and supportive service by giving specialist advice to the individual patient and those important to them with regards to the disease process, treatment interventions and symptom management. Work collaboratively with all members of the Trust's Multidisciplinary teams, our Primary Care colleagues, hospices, community palliative care teams, social services, care homes, district nursing teams and other agencies to provide a seamless integrated pathway for the individual and those important to them. Main duties of the job This post holder will have excellent communication skills in order to co-ordinate and navigate the many complex conversations with patients, those important to them and the wider MDT. Advanced clinical judgement skills to provide advice and recommendations for the management of complex symptoms supported by the wider Palliative care team. The post holder will complete holistic care assessments as per palliative care guidance, involving and communicating with those important to them. Be able to prioritise and advice on each referral to the team. Manage a case load of patients supported by wider team. Role model professional, compassionate behaviour at all times ensuring ward colleagues are supported in order to deliver high quality end of life care.
