Job overview
Aintree Core 24 provides specialist assessment of service users' mental health for those who present to the Emergency Department in times of crisis or are referred from the acute general hospital wards. The role involves the formulation of treatment and risk management plans based on assessment of needs, with appropriate triage and diversion from the service.
The service operates on a shift system of long days, shorter nine and half hour shifts, and nights to provide a 24/7 service, covering 365 days a year. The service has recently undergone organisational changes to increase flexibility across the Urgent Care pathway, supporting Aintree, Royal, Warrington Whiston, and Southport Core 24 services when required.
Main duties of the job
The post holder will:
1. Provide clinical and leadership support for the team operating 24/7.
2. Ensure continuity of care for service users.
3. Undertake triage and assessments, providing enhanced risk assessment and care delivery.
4. Work closely with the Clinical/Operational Leadership Team to ensure effective and safe services.
5. Provide sound clinical supervision to junior staff.
6. Support the development of a skilled and flexible workforce within the Core 24.

Detailed job description and main responsibilities
The post holder will coordinate the care of service users by:
1. Carrying out assessments, evaluation, and safety planning.
2. Developing care plans to meet assessed needs.
3. Liaising with family members and significant others during assessments.
4. Supervising risk assessments and formulating risk management plans.
5. Ensuring compliance with Safeguarding requirements.
6. Participating in setting standards for care based on evidence.
7. Communicating relevant information to the multi-disciplinary team.
8. Documenting service users’ care accurately and legibly.
9. Reacting proactively in service user crises.
10. Participating in team meetings and reflective practice.
