The Role
:
As a Care Coordinator, you'll be maintaining the efficient operation of home care agency and delivering exceptional care to clients.
Responsibilities:
1. Create and sustain client care plans in partnership with clients, families, and healthcare professionals.
2. Arrange and manage caregiver visits, ensuring sufficient staffing levels and appropriate skill alignment.
3. Oversee client well-being and progress, making necessary adjustments to care plans.
4. Perform initial assessments to identify client needs and preferences.
5. Consistently communicate with clients and families about care plans, schedules, and any changes.
6. Assist in the recruitment, onboarding, and training of new caregivers.
7. Keep accurate records of caregiver qualifications, availability, and performance.
Requirements:
Essential:
8. Previously worked as a Care Coordinator or in a similar role.
9. Possess experience in the home care industry or a related field.
10. Strong communication and interpersonal skills.
11. Excellent organisational and time management abilities.
12. Skilled in Microsoft Office and scheduling software.
Desirable:
13. Background in care planning and risk assessments.
14. Qualification in health and social care, such as NVQ Level 3.
15. Understanding of CQC regulations and standards.
16. Valid driving license and access to a vehicle.
