We are looking for passionate and motivated Pharmacy Technicians to join our well-established, friendly and dynamic ward Based Medicine Management Technicians at UHDB.
The Medicine Management technicians play an important role, complementing clinical pharmacists and other members of the wider multi-disciplinary team. The successful applicants will rotate around the different specialty wards at the Royal Derby Hospital, including Oncology, Medicine, T& O, Surgery, Admissions, and the Rehabilitation wards at the Florence Nightingale Community Hospital, allowing the applicant to gain a wealth of experience and knowledge.
Previous experience of working as a pharmacy technician in a hospital or community setting is preferred but not essential.
Main duties of the job
The Medicine Management Technicians will provide a ward-based Medicine Management and Optimisation Service, including medicine reconciliation, patient counselling, and ensuring the quality, safety, and timely supply of discharge prescriptions.
Key responsibilities will include:
* Ensuring the quality, accuracy, and safety of prescribing in exciting clinical areas.
* Supporting the MDT, including pharmacist, doctors, and nursing staff.
* Obtaining detailed and accurate medication histories from multiple sources and reconciling with the inpatient prescription and communicating any discrepancies.
* Communicating detailed information to patients & carers about drug doses, indications, side effects, and efficacy.
* Liaising widely (e.g., CCG or GP staff, patients, carers, etc.) to ensure the timely availability of patient medication.
* Reviewing inpatient and discharge prescriptions to ensure appropriate prescribing and availability of medicines.
* Assessing Patients' own Drugs for suitability for use in the Trust.
* Promoting the use of Trust and national clinical guidelines and optimising patient medication.
* Helping support quality, safety, and timeliness of discharge and facilitating effective communication of medication around admissions and discharges.
* Providing a safe and efficient supply of pharmaceutical products to patients. This may involve dispensing and final checking (ACT) of drug products and compliance aids.
